Patient Care Coordinator Lead:

Philadelphia, PA | Full-Time

Department:  Patient Care Coordination

Hours: Monday-Friday 9a-5p

Key Duties and Responsibilities:   

  • Responsible for managing associates aides and caseloads of up to 40 consumers
  • Review all home visit packets submitted by assessor
  • Ensure all assessments are completed on time and efficiently
  • Submit EIM reports for the team and conduct follow-up calls, emails, etc.
  • Provide and document details of home and any general health/safety concerns
  • Develop care plans, advocate for clients, address need for DME and create emergency plans
  • Conduct follow-ups on APS cases, inactive cases or suspended service cases
  • Perform quarterly assessment re-evaluations, internal employee evaluations, trainings and team building
  • Schedule bi-weekly one on one meetings, bi-weekly team meetings and weekly one on one meetings with new hires
  • Participate in any/ all marketing events
  • Assign new cases in CRM database and document pass/fail areas for aides yearly evaluations
  • Provide support to the Human Resource Department with aide compliance and yearly evaluations
  • Retain case load of clients and aides


  •  Requires a high school diploma or equivalent
  • At least 1 -2 years of experience in a health care or home care environment
  • 2 years of PCC or PCC lead experience preferred
  • Management or supervisory experience required
  • Case management experience preferred
