Patient Care Coordinator:

Philadelphia, PA | Full-Time



Department:  Patient Care Coordination

Hours: Monday-Friday 9a-5p

Job Purpose:  Patient Care Coordinators (PCCs) provide coordinated care to (age-specific) patients by developing, monitoring, and evaluating their home health care plans. The duties of a PCC will vary depending on the assessed needs of the client. The main goal of a PCC is to ensure a high level of care for the consumer given by the home health aides.

Key Duties and Responsibilities:

  • Responsible for managing associated aides and caseloads of up to 50 consumers
  • Initiate 18 calls per day (while in office)
  • Conduct audits of physical file, CRM data entry and employee files
  • Ensure to conduct follow-ups with client and aides
  • Provide and document all details of home visits and general health/ safety concerns
  • Develop a care plan, advocate for clients, address needs of DME and assess emergency care plans
  • Evaluate outcomes of care with supervisor by measuring the effectiveness of active plan and document pass/ fail areas for aide’s yearly evaluations
  • Provide support to Human Resource Department with aide compliance and providing yearly evaluations
  • Manage new aide and participant enrollment


  • High School/GED required
  • 1-3 years of experience in a healthcare related field such as Home Health, Medical equipment or managed care
  • Excellent oral and written communication skills
  • Highly organized and able to prioritize tasks
  • Strong computer skills with proficiency in Microsoft Word & Excel
  • Positive attitude and must be reliable
  • Bi-lingual preferred (Spanish)

Required Licenses/Certifications:  Valid US Driver’s License
